Pokemon's been Junichi Masuda and Go Ichinose pretty much consistently through the portable set. Someone else does the spin-offs.

My favourite Star Wars game soundtracks were the ones for the Old Republic series and Jesse Harlin's Republic Commando soundtrack.

A lot of the other Star Wars games I'd played used excessive amounts of the original movie tracks =P

Nobuo Uematsu for Final Fantasy 6

I have the Grand Finale CD, which is all the tracks performed by an orchestra. It's pretty awesome!
This is their version of "Terra's Theme"

Final Fantasy 6 definitely had the best music of the original games.
