Pretty sure it's just an honorific, and really's got nothing to do with samurai (but they would have been using it then)
like how San means basically Mister, Sama is more like Sir.

Makes sense. I only saw those titles when I was reading Shogun some years ago haven't seen them used in a more conventional setting prior to now.
