Could USBs somehow be adapted to have the benefits Patrick is talking about? Maybe bands could sell little leaflets about themselves which contain a small memory card/chip from which songs can be transferred onto an MP3 player, but which still contains all the tangible advert parts of a CD, without the wasted resources of plastic etc.

Every car audio unit I've seen that has USB will play mp3 files without fuss.  They might need to all be in the root directory, but the solution to that is simple: leave them all in the root directory.  It seems like all you'd need to do is load up a bunch of USB sticks with mp3's of your band, a pdf or two with some promo material ("leaflets"!), and if you really want to get fancy, video in avi or flv format (or both).  Leave everything in the root directory.  Most people will be able to just plug the USB into their car or PC or whatever and play the tunes, probably even play the video(s).  Then pass them out at your gigs.  Buy 'em in bulk and they're down to a couple bucks each.

Spiff:
Using a micro-sd card instead would be even better because you can put them directly in your music player if it has a card slot and if you need a USB-connection you can always buy a micro-sd card reader for small money.

I guess.  If the idea is to put the music on the most "universal" media, sd cards may have surpassed USB sticks by now, I'm not sure.
