If they're just now getting to Sven being the root of Dora's problems, I suspect this may be a new therapist.
Maybe it's the "Ah-ha" of "This girl is going to single-handedly put one of my kids through college".

What I'm imagining is (and we really don't know how many sessions it's been since that first one) that the bulk of the discussion prior to this has simply been All About Sven. After about the third time, though, I think the therapist would start to try to probe through that, in only to try to figure out why she's so fixated on her brother and brother's success.
