A guy with the usual 46,XY chromosomes, but light-to-moderate degree of Androgen Insensitivity. PAIS 2-3 I'd say. I was (mis)diagnosed in 1985 with PAIS 2, based on my physical appearance.
A guy with 46,XX chromosomes, and the most common form of CAH - Congenital Adrenal Hyperplasia. Most 46,XX people with CAH are not as masculinised, and only one in 10 identify as male. There's no good correlation between degree of external masculinisation and gender identity, so some gals look like this, and some guys look more like gals. I was diagnosed in 2005 as having CAH, but only in the last few years was the exact form known (3BHSD - see wiki on the subject).
Now yours truly.... note the general build, especially anomalous wingspan (arms stretching almost to knees), relatively short legs due to premature bone closure, large barrel chest. Lots of "tells" that would cause an endrocrinologist to say "hmmmmm....". I could be that guy's sister . Ironic, he's 46,XX and male, I'm 46,XY (mainly) and female.
