every day -
front right pocket: samsung GS3
front left pocket: wad o' keys, glucose meter, test strips, lancet
back right pocket: wallet
attached to me via tape: enlite cgm sensor and transmitter, the business end of a quickset infusion set.
clipped to my belt: medtronic 530g
on my left wrist: medical ID bracelet

not every day, but becoming more common -
inside my waistband at the ~4:30 position: Hidden Hybrid Holster, Springfield XDm compact in .40 S&W

As 'every single time' as I possibly can, the keys to my flat unless I want to be embarrassingly in the shit with my flatmate.  :-P
I have so many keys on one ring too, so this carries a lot of clout.
Also purse, disposable contact lenses, notepad, phone, earphones, pen, lipbalm, mascara, concealer. That's generally it.

Lots and lots of nicotine lozenges

My phone, my wallet (currently just my credit card, woops) and my house keys (If I'm in the city, if if I'm at home then no keys).
