I long suspected that there is a small and generally-repressed part of Amber that is suicidal. We saw it in her reckless (for her own safety) rescue of Becky. Now, with the very real possibility that someone will finally end all of her problems for good without her even having to do anything personally...? Well, she's almost happy with this outcome.
As for Sal, she's realising just how badly her every escalation has ever gone and she has to deal with the fact that, this time, she's about to murder a mentally-ill woman with only debatable control over her actions.

Because it is fairly easy to go from a choke hold to 'choke to death', especially if the person doing it is angry and not monitoring the other's state. Additionally (and most importantly), I think that Amber wanted it to be 'to death'.

An improperly applied chokehold can easily turn fatal unintentionally. There was a case not so long ago in the States where a police officer put a detainee in a chokehold and the man died shortly thereafter as a result. It's really easy, especially when angry, to do serious damage that way.
That being said, I agree that a lot of readers are blowing it out of proportion and I don't think Sal was ever at serious risk of killing Amber, even if that's what Amber wanted.
