This song has not been performed or recorded by the Vermont band Phish .
It was originally done by Dr. Dre, Snoop, and a NYC hip-hop artist named Phish, and has been covered by a bluegrass band from Texas called the Gourds. The latter circulates on MP3, and is sometimes incorrectly labelled as by Phish.
The hip-hop artist "Phish" is also featured on backup vocals for Dre's "What's the Difference" (track 7 on Dr. Dre 2001 - a title as remarkably unoriginal as using the name Phish nearly two decades after the band started to).

All you folks going nuts over Johnny Cash's version of "Hurt" (as you should be) should check out the rest of the covers he did in the American Series.
Off the top of my head some really good ones were: "One" (U2), "I See a Darkness" (Bonnie 'Prince' Billy), "Rusty Cage" (Soundgarden), "Personal Jesus" (Depeche Mode), "The Mercy Seat" (Nick Cave).
There are loads more, but there's a good start.
