Do you have any idea what you'd like the image to be based around, or even what kind of style you want it in? A good tattoo artist will design something for you for a deposit but they'll need something to go on, otherwise the chances of them coming up with something you're going to want permanently on your body are slim to none. You can get to know tattoo artists in your area by just finding out what tattooists are local (google it or ask on forums) and then just wander in. Tell them what you're thinking of getting done, ask to see their book of tattoos and if they'll answer some questions. Any tattooist that isn't happy to just have a chat with you (when they've got a few minutes) or who doesn't do custom and only works in flash is not a tattooist you want to be working on your body anyway.

As for getting a half or full sleeve as your first tattoo, so long as you talk through with your tattooist what exactly it will involve and you're sure about the design then I say go for it. If there's something small you'd like done as well then it might be an idea to get that first just so you've got a better idea of what you're getting into but if not don't be too worried. It's your tattoo, if you want it get it.

I think it looks way to busy uncoloured.
maybe just a touch of colour would help.

This is the only tattoo I've got.  I've been considering expanding on it for a bit now but can't seem to get it worked out in my head.  I was thinking background of some type then spreading up and over my shoulder (this is on my right shoulder) into a spring/summer/fall type tree tattoo on my right shoulder blade.

EDIT:  if anyone has some suggestions of how to get the pattern/shape of the fish from my shoulder to a piece of paper I'd love you for forever.

I agree completely.
Most people need to look at it for about 10 seconds to figure out what's going on.
Color would make it a lot more coherent, and I love colorful tattoos.
