In my experience, the thong vs. flip-flop thing has as much to do with age gaps as it does regions. My mother and grandparents are still more likely to say thongs than flip flops, and frankly, I can't imagine any of them having a conversation about thong underwear to begin with, so it's not like they ever really had to worry much about potential confusion.
On a related note, I remember when I volunteered at the nursing home and overheard a few old men talking about all the rubbers they went through over the years and their favorite brands. Turns out they meant waders.

Man, I got to "become the best" and instantly clicked "quote" and while isolating that one bit to follow it up with "...the very best? Like no one ever was?" I realized you had already beaten me to it. Damn you.
